Warranty/Repair Form

Before you return your product for repair or for a warranty claim, you must fill out this form in full and send it to our front office. A return email will be sent to you from our office within 2 business days of us receiving your request. If the return is approved you will receive a return authorization # to put on the outside of your box before returning your product.

If the return is not approved our office will have another option available for you when they return contact via email.
